Government institutions are required to provide individuals with a right of access to certain general records and to their own personal information, with limited exceptions.
Immunization Connect Ontario (ICON)
- You can view and print current, overdue and upcoming immunizations. Like your Yellow Card, only better.
- Every time you or your child gets a vaccine, report it here so that you have a complete record of your immunizations and they are submitted to your local Public Health Unit.
