SFO Request for Service/Complaint

From time to time members of the public witness behavior or conditions in their community that violate the Smoke-Free Ontario Act, 2017. This online complaint form offers a quick, convenient, and secure way to direct complaints to our enforcement team at the Heath Unit. You can choose to include your name and contact information if you want to be contacted by the Health Unit to follow up about your complaint, or you can choose to submit your complaint anonymously.

Members of the public, retailers ,vendors and event or activity organizers may have questions about interpretation of the Smoke-Free Ontario Act, 2017 and regulations. This form provides you access to request a site visit, education, or consultation with an inspector under the Act to ensure that your area, premise or event is compliant and your questions answered around the requirements for restricting smoking, vaping of tobacco, cannabis or e-substances that exist under the Smoke-Free Ontario Act 2017.
