Did you receive a notice that your child is missing important vaccines required to attend school? Please review the following information.
Where do I find my child’s vaccine record?
In Ontario, a vaccine record is given to you after getting vaccinated. If you are unable to locate your child’s vaccine record, contact the health care provider, travel clinic, or hospital where the child received vaccines.
Where can I see the vaccine record that is on file at SEPH?
View your child’s previously reported vaccines with Immunization Connect Ontario (ICON).
To verify your child’s immunization ID on ICON, please use the Ontario Immunization ID number on the notice you received or your child’s Ontario health card.

What do I do if my child does not have a health card or a health-care provider?
Students without a health card or health-care provider can book an appointment to receive publicly funded (free) vaccines at SEPH immunization clinics.
