School administrators and licensed child care centre operators are asked to please complete and submit this form if the current absenteeism rate at your school or licensed child care centre is 30% above the typical baseline. A Public Health Nurse or Public Health Inspector will be following up with you in 1-2 business days.
Example of 30% Above Baseline Absenteeism Rates
- In a small school with 20 staff and 80 enrolled students there would be a total of 100 individuals if everyone were present
- To calculate the 30%, the school principal would multiply the total number of individuals by 0.3 (i.e. 100 x 0.3 = 30 individuals)
- This school typically has about 10 individuals absent each day – this is their baseline absenteeism rate
- Therefore, the school would be expected to contact their local PHU and notify the school community via template notification if 40 individuals were absent (baseline of 10 + an additional 30 individuals).
