Chickenpox Reporting Form
The Leeds, Grenville and Lanark District Health Unit is required to report the total number of chickenpox cases by age each month to the Ontario Ministry of Health and Long-Term Care.
Schools may report via the form below, via fax to 613-345-5777 or by phone to 613-345-5685 or 1-800-660-5853, attention Infectious Disease Program, Community Health Protection Department.
AS A REMINDER:
Diarrhea and vomiting, influenza, measles, meningitis, mumps, rubella, and whooping cough (pertussis) must all be reported immediately by telephone to the Leeds, Grenville and Lanark District Health Unit at 613-345-5685 or 1-800-660-5853.
NOT REPORTABLE:
Ear infection, fifth disease, hand, foot & mouth disease, impetigo, mononucleosis, pink eye, and strep throat/scarlet fever.
